In collaboration with the patient and the patients' health care team, the ambulatory pharmacist clinician works to optimize the patient's drug therapy to improve the quality of life and health outcomes of the patient. The standard of care includes ensuring the patient’s medications are appropriate for the patient, effective for the medical condition, safe given the comorbidities and other medications being taken, and able to be taken by the patient as intended. The position requires high level of clinical skills, and the ability to build positive relationships with patients and colleagues, including other health care providers. Essentia Clinical Pharmacy is looking for the right candidate to open a new practice in one of our rural communities, either the Baxter Clinic or Virginia Clinic.Education Qualifications:
Graduation from a pharmacy program accredited by the Accreditation Council for Pharmacy Education (ACPE) or a program outside the U.S. that qualifies the individual to practice in the jurisdiction.
The pharmacist clinician must have completed a PGY1 or PGY2 ambulatory care residency or have at least 2 years experience in ambulatory clinical pharmacy. Consideration for applicants who have completed an ACPE accredited Medication Therapy Management training program.
Applicants must be board certified or become board certified within one year of eligibility to qualify for certification.
Evidence of ongoing clinical practice by proof of 25 patient interactions, reflective of the scope and complexity of privileges requested in the previous 12 months or obtain within 3 months of hire.
Licensure/Certification Qualifications:
- Applicants must be board certified or become board certified within one year of eligibility of an acceptable board specialty certification including: BCACP, BCPS, or BCGP
- Active license to practice pharmacy within MN and WI
- Ambulatory - BLS certified within 90 days from hire date
