Care Management Associate II Nursing

Job Description:

This position supports the RN Care Coordinator by managing patient care plans, utilizing excellent organizational and communication skills, and showing a personal commitment to providing compassionate care.

Education Qualifications:

Key Responsibilities:

  • Manages patient care plans, including developing and updating plans as needed

  • Provides communication with patients, care teams, and other stakeholders to ensure that all relevant information is available to provide the best possible quality of care

  • Assists with scheduling appointments, coordinating care, providing follow-up services to patients, enters orders and authorizations, submits data to health plans, and any other duties as assigned

  • Tracks and maintains patient records, including medical history, medications, and insurance information. Ensure that all patient care plans follow applicable regulations and guidelines. Follow up with patients to ensure their care plans are being followed and that they are receiving the best possible care

  • Provides administrative support including data entry, documentation in the Electronic Medical Record, and other duties as required

  • Collaborates with other departments to ensure the efficient delivery of care

Licensure/Certification Qualifications:

Only candidates that have a Minnesota LPN license or CMA (Certified Medical Assistant) and live in the Essentia Health footprint will be considered for this position.
