Job Description:
Utilizes advanced health assessment, knowledge, and decision making skills, works independently and in collaboration with physicians and other health care professionals. Provides health care to individuals and families, emphasizing health promotion and disease prevention.Education Qualifications:
This position will assist our patients by utilizing a strong team-based approach to care, with a high integration of APP and MDs for the clinic, and inpatient and dialysis units for all 3 phases of nephrology care.
· Desired skill set: Experience in complex chronic disease management
· Typical Shift: up to 0.4 FTE per pay period
· 5% clinic work
· 5% hospital work
· 90% dialysis rounding
· Ideal candidate does not need to live in the Twin Ports area; candidate could live in areas neighboring locations in Northwestern WI.
· Schedule: Variable start and end times due to travel
· Estimated Patients Per Day: 12 (office), 12-15 (dialysis). Adult patients only (18+)
· Other responsibilities: Assist Physicians with non-encounter work
*Hired candidates may be eligible to receive additional compensation in the form of bonuses, quality incentives or production-based compensation.

Licensure/Certification Qualifications:
NP:
Current licensure as a registered nurse in appropriate state.
Current licensure as a Nurse Practitioner in appropriate state.
Current certification in CPR.
National certification in applicable area.
Successful completion of Essentia Health credentialing process prior to practice
PA:
Current licensure as a physician assistant in appropriate state.
Current certification in CPR.
National certification.
Successful completion of Essentia Health credentialing process prior to practice
