CHW - Community Health Worker (FT Days) - West Duluth Clinic

Job Description:
We are looking for someone with the natural gift of caring to work as a full-time Community Health Worker. CHWs are natural helpers and individuals who have a close understanding of the community served. They are specifically hired and trained to address behavioral and social determinants of health through patient-centered goal setting. CHWs reach beyond clinic walls to help patients meet their goals. They creatively leverage patient, family, and community resources to address root causes of patient’s health concerns.

Education Qualifications:

• Bachelor's degree is preferred.

Licensure/Certification Qualifications:

• Community Health Worker (CHW) certificate is preferred.
• 1-2 years’ experience in social service, healthcare (including behavioral health) setting or related field is required.
